Документы, необходимые для прохождения медицинского осмотра по приказам 428 и 29Н:
- Направление с места работы на прохождение осмотра;
- Справка от врача-нарколога Выборгского наркологического диспансера ( Кленовая ул., 2, тел./факс (81378) 2-81-00);
- Справка от врача-психиатра* Психоневрологического Диспансера (Куйбышева ул., 7, тел.: (81378) 2-50-75);
*выдается только при наличии энцефалограммы.
- Энцефалограмма (делается бесплатно в составе комиссии для работников ОАО «РЖД»).
Документы, необходимые для прохождения медицинского осмотра по Приказу 29Н:
- Направление с места работы на прохождение осмотра (возможно предоставление справок).
- запись на комиссию осуществляется строго с 0800 до 1000 в кабинете №7.

- ПРИКАЗ от 29 марта 1999 г. N 6Ц "О порядке проведения предварительных, при поступлении на работу, и периодических медицинских осмотров на федеральном железнодорожном транспорте";
- ПРИКАЗ от 12 апреля 2011 г. N 302н "Об утверждении перечней вредных и (или) опасных производственных факторов и работ, при выполнении которых проводятся обязательные предварительные и периодические медицинские осмотры (обследования), и порядка проведения обязательных предварительных и периодических медицинских осмотров (обследований) работников, занятых на тяжелых работах и на работах с вредными и (или) опасными условиями труда.
